Healthcare centres or units involved in the management of patients with established or high risk for atherosclerotic cardiovascular disease (ASCVD) that have available written protocols to encourage and facilitate disease self-measurement for patients with hypertension and/or diabetes.
Denominator
This is a binary measure. The target population is all healthcare centres involved in the management of patients with established or high risk for ASCVD.
