Defining Attributes

Definition

Percentage of new inpatients at a rehabilitation ward or unit receiving an individualised care plan for rehabilitation. The development of an individualised rehabilitation care plan by the rehabilitation facility can be based on a national standard. An individualised rehabilitation care plan should be developed for every client admitted at a dedicated ward and usually includes: i) a functioning assessment at the start of the rehabilitation episode; ii) person-centred goal-setting; and iii) an intervention plan.
