Defining Attributes

Definition

Percentage of patients aged 18 years and older with a diagnosis of ischaemic stroke or spontaneous intracerebral haemorrhage or subarachnoid haemorrhage who were assessed for the need for occupational, physical, or speech rehabilitation services at or prior to acute inpatient discharge and screening results were used to determine referral recommendation to appropriate next level of care, or documentation that no rehabilitation is necessary. Updated in 2016.
