Definition
Percentage of cardiac rehabilitation (CR) patients who were provided an intervention to promote long-term physical activity post-CR.
Numerator
The total number of patients who completed the program who have documentation they were given an intervention (either within or outside) designed to promote sustained physical activity (at guideline target of 150 minutes minimum) after completing CR. To complete the CR program a patient must have attended at least some of the CR intervention components and have had a formal re-assessment by the CR team at the conclusion of the CR intervention.
