Percentage of patients with an acute stroke who are transferred to inpatient rehabilitation services within 7 days of acute admission with primary stroke diagnosis.

Numerator

Number of patients admitted with acute stroke who are transferred to inpatient rehabilitation services within 7 days of acute admission.

Denominator

Number of patients discharged from an acute hospital event with a primary stroke diagnosis (i.e. ICD10 codes I61, I63, I64) who are immediately transferred to inpatient rehabilitation.
