Definition
Percentage of cardiac rehabilitation (CR) patients with suspected clinical depression who were referred for mental health management.
Numerator
The total number of enrolled patients who screened positively for possible depression at any point during CR who were offered referral to an appropriately-qualified mental health professional or family physician (either within or outside the program). Enrolment is defined as patient attendance at a first CR program visit. 'Suspected clinical depression' is defined as either (i) a score on a self-report measure of depression that exceeds a specified threshold (or 'caseness score') (e.g., ≥8 on the Depression subscale of the Hospital Anxiety and Depression Scale), or (ii) diagnosis of clinical depression following the administration of a structured clinical interview (e.g., Structured Clinical Interview for DSM-IV-TR or MINI International Neuropsychiatric Interview). An internal referral is defined as a referral to a member of the interdisciplinary CR team (e.g., psychologist, counsellor, social worker). An external referral is defined as a referral to a mental health professional who is not part of the CR program.
Denominator
The total number of patients who screened positive for possible depression at any point during CR. Program discharge is defined as patient participated in at least some of the program and the file was closed out by the CR team.

Denominators, Exclusions: • Patients who have already accessed care from a mental health professional before CR initiation.
