Defining Attributes

Definition

Percentage of cardiac rehabilitation (CR) patients meeting the target amount of 150 minutes of physical activity per week at program completion.

Numerator

The total number of patients at CR completion that upon validated assessment of physical activity, meets guideline target (150 minutes of moderate to vigorous physical activity per week). To complete the CR program a patient must have attended at least some of the CR intervention components and have had a formal re-assessment by the CR team at the conclusion of the CR intervention.

Denominator

The total number of CR patients who completed the program.
