Defining Attributes

Definition

Percentage of cardiac rehabilitation (CR) discharge summaries that include the recommended elements.

Numerator

The number of patient discharge summaries that include all of the following: 1. a description of the patient's lifestyle risk factors (physical activity, diet and smoking as relevant), 2. medical risk factors (blood pressure, lipids and glucose as relevant), 3. cardiac medications, and 4. and long-term management goals. Program discharge is defined as patient participated in at least some of the program and the file was closed out by the CR team.

Denominator

The total number of patients discharged from the CR program. Program discharge is defined as patient participated in at least some of the program and the file was closed out by the CR team.

Exclusions

Denominators, Exclusions: • Patients who died before being discharged from CR program. • Patients who are re-admitted to hospital before being discharged from CR program.
