Definition

Percentage of cardiac rehabilitation (CR) patients with documented communication which outlines patient risk factors and an ongoing management plan between the CR program and the ongoing care provider (e.g., general practitioner).

Numerator

The total number of patients who completed a CR program with documented communication and ongoing management plan from the CR program to at least one ongoing care provider (e.g., general practitioner).

Denominator

The total number of patients who completed the CR program.
