Definition

Percentage of residents with an end of life care plan (advanced care planning).

Numerator

Number of residents with an end-of-life care plan, in accordance with the wishes of the resident. Has three criteria and if all three criteria are met, the resident counts towards this indicator. 1. Has a proposal been made to the resident to have a representative. 2. Are the resident's wishes regarding end-of-life care discussed? 3. Based on the resident's wishes regarding the care surrounding the end of life a care plan has been drawn up?

Denominator

Total number of residents present in the residential care centre on the registration day.

Exclusions

Short stay residents.
