Definition
Percentage of long-stay residents who had a weight loss of 5% or more in the last month or 10% or more in the last 6 months who were not on a physician prescribed weight-loss regimen noted in an MDS assessment during the selected quarter.
Numerator
Long-stay nursing home residents with a selected target assessment which indicates a weight loss of 5% or more in the last month or 10% or more in the last 6 months who were not on a physician prescribed weight-loss regimen.
Denominator
Long-stay nursing home residents with a selected target assessment except those with exclusions.
Exclusions
Target assessment is an OBRA Admission assessment or a 5-Day assessment, prognosis of life expectancy is less than 6 months, receiving hospice care, weight loss item is missing.
