Definition

Percent of residents in the past six months with unintentional weight loss of 10% or more in 6 months.

Numerator

Number of residents in the past six months with unintentional one had weight loss of 10% or more.

Denominator

Total number of residents with 2 valid weight measurements (baseline and 6 month) during the registration period.

Exclusions

At the time of weighing, the resident is taking acute therapy of diuretics and these treatment lasts for a maximum of 30 days; on a weight-loss diet (low-calorie diet); is assessed as terminal by the doctor and the care team (prognosis three months of life); in too much pain to be weighed. Short stay residents.
