Definition
Percentage of residents whose mid-loss activities of daily living (ADL) functioning (transfer and locomotion) worsened or remained completely dependent in mid-loss ADL.
Numerator
Residents with worsened mid-loss activities of daily living (ADL) self performance on their target assessment compared with their previous assessment or were completely dependent in mid-loss ADLs on both target and prior assessment.
Denominator
All residents (must have assessment in previous quarter).
Exclusions
Residents who are comatose, end-stage (6 months or less to live) or receiving hospice / palliative care.

Individual Covariates: Not totally dependent in transferring, Locomotion problem, Personal Severity Index (PSI): Subset 2—Non-Diagnoses, Age younger than 65.
