Based on the question: "The following questions concern access to treatment during your most recent appointment, mentioned above", presented below as, for instance "Were you able to contact the place of care without difficulty? The response options were: 1) absolutely yes, 2) to some extent, 3) not really, 4) absolutely not, 5) cannot say
