Defining Attributes

Definition

The team has a complete register of all patients for whom they are providing end of life care. This register should include: Name of carer, diagnosis (+ code), GP name, problems/concerns, anticipated needs, information given/carer issues, DS 1500 date, CNS, hospice/SPC, OOH handover form date sent, and preferred place of care stated + date.

Numerator

Number of patients with fully completed details.

Denominator

Number of patients on the caseload receiving end of life care.

Exclusions

Patients on the caseload for end of life care who have had < 3 visits from DN service.
