Definition

All patients should have a common assessment framework carried out within three weeks of the date of the first district nursing visit.

Numerator

Number of patients with documented common assessment framework.

Denominator

Number of patients on caseload with three or more visits.

Exclusions

Patients who have less than 3 visits e.g. to check on healing of surgical wound.
