Defining Attributes

Definition

Percentage of patients who have their most recent HbA1c result missing (e.g., test performed but result not recorded in patient record), or if the most recent HbA1c result is greater than 9 percent, or if no HbA1c tests were performed during the measurement period.

Numerator

Patients if the most recent HbA1c result is missing (e.g., test performed but result not recorded in patient record), or if the most recent HbA1c result is greater than 9 percent, or if no HbA1c tests were performed during the measurement period.

Denominator

Patients who are 18–75 years of age during the measurement period, whose diagnosis of diabetes is active during the measurement period, and who have an eligible encounter.

Exclusions

Patients who received hospice services during the measurement period, patients who are 66 and older with advanced illness and frailty, and patients who are 66 and older who are living long-term in an institution for more than 90 consecutive days during the measurement period. Exclude patients receiving palliative care during the measurement period.
