Definition

The Consumer Assessment of Healthcare Providers and Systems Clinician & Group Survey 3.0 (CG-CAHPS) is a standardised survey instrument that asks patients to report on their experiences with primary or specialty care received from providers and their staff in ambulatory care settings over the preceding 6 months. The CG-CAHPS 3.0 survey can be used in both primary care and specialty care settings. The adult survey is administered to patients aged 18 and over. Patients who had at least one visit to a selected provider during the past 6 months are eligible to be surveyed.

Numerator

Full and partial survey completes, defined as follows: a survey is a full complete if responses are available for at least 16 of the 32 key survey items and at least one reportable performance-based adjustment (PBA) item. A partially completed survey includes responses answered for at least one reportable PBA item and fewer than 16 of the 32 key items. The percentage of survey responses answering each PBA item in the most positive way is computed.

Denominator

All patients regardless of payer or insurance, who are 18 or older at the time of roster creation and are not deceased and have been seen at least once by a primary care practitioner at the practice site address listed in the PCF practitioner roster. Includes all patients with a visit (which may be in-person or telehealth) that occurred since January 1 of the performance year.
