Definition
This is a composite measure that assesses an individual´s knowledge, skills and confidence for managing their health and health care.
Numerator
Patients with a Baseline PAM® score and then a second score taken within 12 months of the baseline (but not less than 6 months).
Denominator
Patients aged 14 years and older with a qualifying visit at least once during the performance period.
Exclusions
Patients with a diagnosis of dementia, Huntington's disease, cognitive impairment or Alzheimer's disease.
