Definition
Percentage of patients ≥18 to ≤85 years of age with hypertension who had a nephropathy screening test or evidence of nephropathy during the reporting quarter or in the 09 months prior to the reporting quarter.
Numerator
Patients in the denominator population with a screening for nephropathy or evidence of nephropathy within 12 months (prior to the end of reporting quarter).
Denominator
Total number of unique patients ≥18 to ≤85 years of age, with hypertension related outpatient visit/s during the reporting quarter AND who had at least 2 hypertension related outpatient visits within 09 months (prior to the reporting quarter).
Exclusions
Documentation of kidney transplant (within the denominator time-frame), pregnancy (during the reporting quarter) and all ABM Mandate encounters.
