Percentage of patient outpatient visits by primary care physicians that resulted in a referral for services or a specialist/consultant visit during the reporting quarter.
Numerator
Number of outpatient visits that resulted in a referral to the specialists/consultants.
Denominator
Total numbers of outpatient visits by the primary care physicians during the reporting quarter.
Exclusions
Referrals to same specialty (e.g. Family Medicine to Family Medicine/GP clinics), dental visits and all ABM Mandate encounters.

Centrally collected claim data (KEH) and patient medical record. Each provider will nominate one member of staff to coordinate, collect and monitor primary care quality indicators. Primary care provider is required to submit quarterly submission of data through Jawda e-notification system.
