Defining Attributes

Definition

Percentage of patients ≥18 years of age with positive screening for depression had a follow-up plan documented on the date of the eligible encounter.

Numerator

Patients from the denominator who had a follow-up plan documented on the date of the encounter or who received follow-up care within 30 days of a positive depression screening finding OR Follow-up for a positive depression screening must include one or more of the following: (1) Referral to a practitioner who is qualified to diagnose and treat depression; (2) Pharmacological interventions; (3) Other interventions or follow-up for the diagnosis or treatment of depression.

Denominator

Total number of unique patients aged ≥18 years of age at the beginning of the reporting quarter who were positive for depression screening during the reporting quarter OR Patients who have been diagnosed with depression.

Exclusions

Screening for depression is documented as negative, a follow-up plan is not required; negative screening for depression; patient refuses for the follow up; individuals who do not qualify for insurance benefits and all ABM Mandate encounters.

Centrally collected claim data (KEH) and patient medical record. Each provider will nominate one member of staff to coordinate, collect and monitor primary care quality indicators. Primary care provider is required to submit quarterly submission of data through Jawda e-notification system.
