Definition

Percentage of actively enrolled home-based primary care and palliative care patients who received an assessment of their cognitive ability.

Numerator

Number of newly enrolled (and established) home-based primary care and palliative care patients for whom cognitive assessment was performed (annually).

Denominator

Total number of newly enrolled and established (and active) home-based primary care and palliative care patients.

Exclusions

Most recent new patient encounter (with subsequent established patient encounter) occurs within the last 90 days of the measurement period. Documentation of medical reason(s) for not assessing cognition (eg, patient with very advanced stage dementia, other medical reason) or Documentation of patient reason(s) for not assessing cognition.
