Defining Attributes

Definition

The proportion of people who saw three or more health professionals in the last 12 months for the same condition and who reported that issues were caused by a lack of communication between the health professionals. Based on the question, "Were there any issues caused by a lack of communication or co-ordination among the health professionals?" The response options included: 1. Yes, 5. No, 6. Can't remember.
