The end-of-life questionnaire (ELQ) is the main source of register data in the Swedish Register of Palliative Care (SRPC) for patients dying in hospitals and other care settings in Sweden. Information is extracted from the medical records using a 27-item questionnaire and registered retrospectively online by a staff member involved in the EOL care of the particular patient.
