Question: How often in the last month of life did a pre-planned multidisciplinary consultation take place (face-to-face or via phone) between the care providers to discuss the care objectives and/or options in terms of palliative care? Answer: No such consultation/once in the last month of life/approximately once a week/ approximately everyday - Positive score: If the multidisciplinary consultation occurred 'once a week' or 'once a day or more'
