Based on data collected from Swiss RAI-HC, an adapted and shorter version of the original and internationally used English-language interRAI-HC. Swiss RAI-HC data are measured within a 90-day period at entry to home care services (baseline) and calculated six monthly unless there is a significant clinical change.
