Definition
Percentage of home health quality episodes during which the patient demonstrates an improvement in the condition of surgical wounds.
Numerator
Number of home health quality episodes where the patient has a better status of surgical wounds at discharge compared to start (resumption) of care.
Denominator
Number of home health quality episodes ending with a discharge during the reporting period, other than those covered by generic or measure-specific exclusions.
Exclusions
Home health episodes of care for which the patient at the start/resumption of care was not short of breath at any time episodes that end with inpatient facility transfer or death.
