Numerator
A subset of the denominator that represents patients age 18 and older who responded to a question about post hospital symptom monitoring explained by staff.
Denominator
Patients age 18 and older who responded to questions regarding information in writing about what symptoms or health problems to look out for after discharge, in the Canadian Institute for Health Information- Patient Experiences Survey
Exclusions
Patient populations not included in maternal, surgical and medical. (To ensure comparability across hospitals and to prevent misinterpretation, results do not include maternity patients).
