Definition
The extent to which staff are treated fairly when they make mistakes and there is a focus on learning from mistakes and supporting staff involved in errors.
Numerator
The number of "strongly agree" or "agree" responses to Item A10, and the number of "strongly disagree" or "disagree" responses to Items A6, A7 and A13.
Denominator
The total number of responses to each item. The item-level percent positive scores are divided by 4 to produce the composite measure score.
Exclusions
Response option "Does not apply or don't know' and missing responses are excluded from calculations.
