Definition
The extent to which staff are informed when errors occur, discuss ways to prevent errors, and are informed when changes are made.
Numerator
The number of "always" or "most of the time" responses to Items C1, C2 and C3.
Denominator
The total number of responses to each item. The item-level percent positive scores are divided by 3 to produce the composite measure score.
Exclusions
