Number of people with dementia with a registered personalised care plan, shared between the person with dementia, informal caregiver and care professional, that is periodically updated. This means that: At least two types of interventions were discussed with and offered to the person with dementia, The care plan is drawn up in agreement with person with dementia, informal caregiver and professional at the service, Response to initiated interventions has been monitored less than one year ago.
