Definition
The proportion of long-term facility stays among Medicaid Managed Long-Term Services and Supports (MLTSS) participants age 18 and older that result in successful transitions to the community (community residence for 60 or more days).
Numerator
The count of discharges from a facility to the community from July 1 of the year prior to the measurement year through October 31 of the measurement year that result in successful transition to the community for 60 consecutive days Discharges that result in death, hospitalisation, or readmission to the facility within 60 days of discharge from the facility do not meet the numerator criteria.
Denominator
Number of long-term facility stays among Medicaid Managed Long-Term Services and Supports participants age 18 and older.
