Defining Attributes

Definition

The proportion of admissions to a facility among Medicaid Managed Long-Term Services and Supports (MLTSS) participants age 18 and older that result in successful discharge to the community (community residence for 60 or more days) within 100 days of admission

Numerator

The count of discharges from a facility to the community during the measurement year that occurred within 100 days or fewer of admission Discharges that result in death, hospitalisation, or readmission to the facility within 60 days of discharge from the facility do not meet the numerator criteria.

Denominator

Admissions to a facility among Medicaid Managed Long-Term Services and Supports participants age 18 and older (inferred)
