Defining Attributes

Definition

Percentage of admissions with an unplanned readmissions for any reason within 30 days of discharge from the hospital. A hospital discharge was identified as having a readmission if the beneficiary was readmitted to a hospital within 30 days, either for the condition associated with the index hospital stay or for a different reason.

Numerator

Number of unplanned readmissions for any reason within 30 days of discharge from the hospital (inferred).

Denominator

Patients age 18 and older discharged alive from the hospital

Exclusions

Planned readmissions such as for chemotherapy
