Definition
Percentage of patients who have had a 7-day post-hospital discharge follow-up by a primary care provider (physician or nurse practitioner) for the following conditions: pneumonia, diabetes, stroke, gastrointestinal disease, congestive heart failure, chronic obstructive pulmonary disease, and cardiac conditions.
Numerator
Number of hospital discharges where the patient was seen by a primary care provider (physician or nurse practitioner) within 7 days of discharge for the following conditions: pneumonia, diabetes, stroke, gastrointestinal disease, congestive heart failure, chronic obstructive pulmonary disease, and cardiac conditions.
Denominator
Number of acute care discharges for an episode of care in which one of the mentioned conditions is recorded in the first hospitalisation of the episode within each fiscal year (minus 30 days for follow-up).
