Definition

Proportion of MHA-related hospital discharges where the patient was seen by a primary care provider, psychiatrist and/or paediatrician within 7 days

Numerator

Consults/ visits with primary care providers, psychiatrists and/or paediatricians occurring within 7 days from discharge taking place in an office, home or long-term care setting. Patients readmitted to hospital or that died within 30 days of discharge

Denominator

Patients discharged alive from a hospital in Ontario for mental health and addictions in the reporting period

Exclusions

Patients readmitted to hospital or that died within 30 days of discharge
