Defining Attributes

Definition

Percentage of people under adult mental illness specialties on Care Programme Approach (CPA) who were followed up (either by face to face contact or by phone discussion) within 7 days of discharge from psychiatric inpatient care during the quarter. All patients discharge to their place of residence, care home, residential accommodation, or to non psychiatric care must be followed up within 7 days of discharge. The seven-day period should be measured in days, not hours, and should start on the day after the discharge.

Numerator

The number of patients who were followed up either by face to face contact or by a phone discussion within 7 days of discharge from psychiatric inpatient care.

Denominator

The total number of people under adult mental illness specialties on CPA discharged from psychiatric inpatient care during the quarter (QA). All patients discharged from a psychiatric inpatient ward are regarded as being on CPA.

Exclusions

Patients who die within 7 days of discharge; Where legal precedence has forced the removal of a patient from the country; Patients transferred to NHS psychiatric inpatient ward; CAMHS (child and adolescent mental health services).
