Residents >65 years of age were tracked during acute care transitions from their LTC facility to participating EDs, and back, using a Transitions Tracking Tool (T3) developed by the research team. Trained research assistants (RAs) identified all resident transitions from participating LTC facilities and EDs using ED medical records. RAs collected medical record data across settings for all transitions. LTC healthcare providers were contacted to identify changes in resident condition after emergency transition.
