Definition
Percentage of outpatients with a diagnosis of heart failure with a reduced ejection fraction (HFrEF) of ≤40% who are referred to outpatient cardiac rehabilitation.
Numerator
Number of outpatients who have been referred to an outpatient cardiac rehabilitation program which includes the following: Communication between the healthcare provider and the patient of the recommendation to attend an outpatient cardiac rehabilitation program AND referral sent to outpatient cardiac rehabilitation program.
Denominator
Outpatients with a diagnosis of heart failure with a reduced ejection fraction (HFrEF) of ≤40 %.
Exclusions
