Percentage of discharges for members 18 years of age and older who had documentation of medication reconciliation on the date of discharge through 30 days after discharge.
Numerator
Number in the denominator with documentation of medication reconciliation on the date of discharge through 30 days after discharge (31 total days).
Denominator
Acute or non-acute inpatient discharges for Medicare beneficiaries 18 years and older.
Exclusions
Members in hospice, members that do not have continuous enrolment from the date of discharge through 30 days after discharge (31 total days). Exclude both the initial and the readmission/direct transfer discharge if the last discharge occurs after December 1 of the measurement year.
