Defining Attributes

Definition

Percentage of all live discharges from hospice that were followed by hospitalisation within two days, and where the patient also died during the inpatient hospitalisation stay. Part of the Hospice Care Index (HCI) Indicator where 1 point is earned if the hospice's individual hospice score for Type 2 burdensome transitions falls below the 90th percentile ranking among hospices nationally.

Numerator

The total number of live discharges from the hospice followed by a hospitalisation within two days of live discharge with death in the hospital within a reporting period.

Denominator

The total number of all live discharges from the hospice within a reporting period.

Exclusions

Hospices with fewer than 20 discharges in the year are not assigned a calculated index score.
