Numerator

Count of acute inpatient discharges where a follow up community contact (for the same person) exists where: Community follow-up activity start date is between 1 and 7 days after acute inpatient discharge date.

The data is collected from district health boards (DHBs) and non-governmental organisations (NGOs). It is stored in the PRIMHD datamart which is part of the Ministry's national data warehouse.
