The rating reflects patients' feedback on whether hospital staff provided adequate information and support on discharge .
The extent to which staff are informed when errors occur, discuss ways to prevent errors, and are informed when changes are made.
The rating reflects patients' feedback on how often hospital staff explained the purpose of any new medicine and what side effects that medicine might have.
The extent to which staff speak up if they see something unsafe and feel comfortable asking questions.
The rating summarises how well hospital staff communicate with patients about the help they would need at home after leaving the hospital, including written information about symptoms or health problems to watch for during their recovery.
The extent to which hospital management shows that patient safety is a top priority and provides adequate resources for patient safety.
The weighted average of the observed-to-expected ratios for the mortality rate for the following inpatient conditions: Acute Myocardial Infarction (AMI), Heart Failure, Acute Stroke, Gastrointestinal Haemorrhage, Hip Fracture and Pneumonia.
The weighted average of the observed-to-expected ratios for the mortality rate for the following inpatient procedures: Esophageal Resection, Pancreatic Resection, Abdominal Aortic Aneurysm (AAA) Repair, Coronary Artery Bypass Graft (CABG), Percutaneous Coronary Intervention (PCI) and Carotid Endarterectomy.
The extent to which work processes are regularly reviewed, changes are made to keep mistakes from happening again, and changes are evaluated.
