Definition
Percentage of designated supportive living or long-term care residents that returned to the emergency department within 72 hours or 30 days of their initial or previous visit to the emergency department.
Numerator
Total number of emergency department and urgent care centre visits which occurred between 6 hours of initial discharge and 72 hours or 30 days thereafter for designated supportive living or long-term care residents.
Denominator
Total number of initial emergency department/urgent care centre discharges for designated supportive living or long-term care residents.
Exclusions
Any visits which resulted in admission/transfer to hospital, death.
